National Disability Insurance Scheme (NDIS) Health Toolkit
This toolkit has been developed through a funded partnership between Deakin University, G21 and Barwon Health which was tasked with exploring the interface between the NDIS and the health sector within the Barwon region. The project was funded by the NDIA Community Inclusion and Capacity Development Grants Programme and took place in 2016-2017.

The project identified key issues at the health/NDIS interface and developed tools for people working in the health sector. The tools were developed with a view to improving the interface with the NDIS and improve both the experience and outcomes for people with a disability. 

The project team developed a website as one of the resources which include a range of templates that can be used by allied health professionals.
